Foundation · $3,000 per year

A focused first look, and a physician who stays with you.

Your membership
  • 4 physician visits on a set schedule — months 1, 3, 6 and 9. Additional visits available at cost.
  • Direct email and text access to your care team.
  • Access to advanced therapeutics.
  • Built around a structured visit schedule — the most defined cadence of the three.
Your included assessment
  • DEXA scan + your Somaentis DEXA Report — lean mass, body fat, visceral fat, fat distribution, and a total-body bone-density screen.
  • Resting Metabolic Rate, reviewed one-to-one with our Metabolic Expert.
  • Core cardiometabolic blood panel — lipids and ApoB, glucose, insulin, HbA1c, hs-CRP, CMP and CBC.
  • A focused Somaentis Medical Report with a personalized protocol, reviewed by your physician. Your blood results are interpreted inside the report — not handed to you as a lab sheet.

Assessment value: $1,200. Included in your membership. The blood panel is billed to your insurance (co-pay applies), or $450 if you are uninsured.

Performance · $5,500 per year

The complete picture — measured, explained, and re-measured.

  • Repeat DEXA, VO₂Max, RMR and VALD at six months — the same tests, so your progress is measured, not estimated.
  • Two reports: your Integrated Longevity & Performance Report and your Somaentis Medical Report, which carries your personalized longevity protocol.

Your included assessment — five divisions
  • 01 Metabolic — resting metabolic rate, continuous glucose monitoring, and 100+ blood biomarkers spanning glucose and insulin, lipids and ApoB, hormones, inflammation and organ function.
  • 02 Structural — DEXA body composition and a physician-led physical exam.
  • 03 Functional — HumanTrak and Dynamo movement, balance and strength testing; VO₂Max with your precise training zones.
  • 04 Vascular — blood pressure and cardiovascular exam, carotid ultrasound, coronary CT angiogram and calcium score, EKG, and cardiovascular blood markers.
  • 05 Cognitive-Motor-Sleep — Creyos cognitive baseline, at-home sleep study, and balance testing.

Assessment value: $3,900. Included in your membership. Coronary CT is performed at our partner imaging facility and included.

Executive · $7,500 per year
